Your First Visit at Dennis Family Wellness Center — Here Is Everything You Need to Know

You scheduled your appointment. The hard part is done. This page walks you through everything before you arrive — forms, what to bring, what to expect, and how to find us.

Step One — Complete Your Intake Forms Online

The fastest way to get through your first visit is to complete your paperwork before you arrive. Our online intake system lets you submit your health history, insurance information, and consent forms from any device — phone, tablet, or computer — at your convenience.

It takes most patients 10 to 15 minutes to complete. Finishing it ahead of time means your first visit is focused entirely on your examination and care — not paperwork.

If you have any difficulty with the online forms or would prefer to complete them in the office, plan to arrive 15 minutes before your appointment time. We will have everything ready for you at the front desk.

What to Bring to Your First Visit

FOR ALL NEW PATIENTS:

  • Photo ID (driver’s license or state ID)
  • Insurance card — front and back
  • Any imaging you already have: X-rays, MRI, CT scan reports — physical films or digital records both accepted
  • A list of current medications and supplements if you have one
  • Any prior treatment records relevant to your current complaint — helpful but not required
  • Comfortable, loose clothing — you may be asked to move through ranges of motion during your examination

 

IF YOUR VISIT IS COVERED BY HEALTH INSURANCE:
Bring both your primary insurance card and any secondary coverage cards. We will verify your benefits before treatment begins and explain your coverage clearly.

If You Were Injured in a Car Accident — What to Bring

Car accident cases involve additional documentation. Bringing the right information to your first visit allows Dr. Dennis to begin proper clinical documentation from day one — which matters significantly for your recovery and for any personal injury claim.

Please bring or have available:

  • Your auto insurance card and policy number
  • The at-fault driver’s insurance information (carrier name, policy number, adjuster name and phone if you have it)
  • Your claim number — if one has been assigned by the insurance company
  • Your attorney’s name and contact information — if you are already working with a personal injury attorney
  • The accident report or case number from the police report — if one was filed
  • Any photos of vehicle damage or visible injuries — helpful for documentation
  • A brief written timeline of the accident and your symptoms since — optional but useful

Note: If you do not have all of this information yet, do not delay your visit. Dr. Dennis can begin your evaluation with whatever you have available. The most important thing is to be seen promptly. Delays in evaluation after an accident create gaps in the clinical record that are difficult to correct later.
